They are saying now that runners from Ebola afflicted countries will not be allowed to run in the New York marathon. But does that really mean anything? Oh my! Well, it appears the "trust me" policies are not sufficiently reliable unfortunately, even for health care workers, and even for doctors.

So far, it seems to me that a person really isn't contagious at least until they have a fever, but someone else needs to go to them a take the temperature and record it at least twice a day. With a fever, the person should be immediately isolated, pending test results.

Thanks for that info SueZ. I think it is crucial. Here is a quote from the study cited by the article:

"The most common symptoms reported between symptom onset and case detection included fever (87.1%), fatigue (76.4%), loss of appetite (64.5%), vomiting (67.6%), diarrhea (65.6%), headache (53.4%), and abdominal pain (44.3%). Specific hemorrhagic symptoms were rarely reported (in <1% to 5.7% of patients). “Unexplained bleeding,” however, was reported in 18.0% of cases. These patterns are similar in each country."

So there's where the 13% comes in (100% - 87.1%)

But I also found an editorial (October 27) from the New England Journal of Medicine which is where the original study was published:

"The governors of a number of states, including New York and New Jersey, recently imposed 21-day quarantines on health care workers returning to the United States from regions of the world where they may have cared for patients with Ebola virus disease. We understand their motivation for this policy — to protect the citizens of their states from contracting this often-fatal illness. This approach, however, is not scientifically based, is unfair and unwise, and will impede essential efforts to stop these awful outbreaks of Ebola disease at their source, which is the only satisfactory goal. The governors' action is like driving a carpet tack with a sledgehammer: it gets the job done but overall is more destructive than beneficial.

"Health care professionals treating patients with this illness have learned that transmission arises from contact with bodily fluids of a person who is symptomatic — that is, has a fever, vomiting, diarrhea, and malaise. We have very strong reason to believe that transmission occurs when the viral load in bodily fluids is high, on the order of millions of virions per microliter. This recognition has led to the dictum that an asymptomatic person is not contagious; field experience in West Africa has shown that conclusion to be valid. Therefore, an asymptomatic health care worker returning from treating patients with Ebola, even if he or she were infected, would not be contagious. Maybe it all means that a person might not have a fever, but they would still have to be symptomatic with diarrhea, vomiting, etc. before they could transmit the disease. It does seem so far that infection requires contact with body fluids.

If this is correct, we need to rely on detecting any of the symptoms - not just fever.

Without quarantine then, we're back to "trust me". But if someone started having these more serious symptoms, wouldn't they want to seek help? I don't know if we can say they always would...

So how do we get more help into Africa and still include the quarantine? Difficult issue.

SueZ, another thought:

Maybe 13% of Ebola patients never had their temperature taken. And maybe 100% of patients being monitored for Ebola did have their temperature taken; and in those cases, fever always preceded other symptoms.

It seems likely that not all Ebola patients staying at home would even have thermometers; so if asked, they or their surviving relatives wouldn't be able to confirm whether they ever had a fever or not.

I have no idea though. This is just a guess, and I haven't read the entire study. Maybe there is more detail about how they got their figures.
